Death Certificate Service in Bangalore
Death certificate is an important document for many purposes such as property inheritance, bank deposit transfers, widow pensions, insurance cases etc. Legally it is used in settling estates, selling stocks, deciding pension benefits, and providing evidence in court.
It is advisable to register the deaths within 21 days of the event so that there is not much process involved in obtaining the certificate.

Required Documents
- Proof of birth of the deceased
- Aadhaar card of the deceased
- ID card of the applicant
- Residential Proof
- Application form for Death Certificate : An affidavit specifying the date and time of death or death slip by doctor
- Receipt from crematorium or burial ground.
Procedure
If a death occurs at a hospital, a relative should fill Form 2, which the hospital will then send to the BBMP office. The hospital will give the Cause of Death Certificate free-of-cost as soon as the death occurs. The MOH will provide the death certificate, which can then be collected at a specified date.
If death occurs at home, Form 2 can be filled later at the BBMP zonal office. Applicants have to submit documents including the Cause of Death and cremation certificates, after which the registrar conducts an inquiry and the certificate is issued.
